changeing of home address

Sir I am a postal life insuarance policy holder and presently a govt job in bsf , as per our bound paper my home address is required to be change but no any instruction /policy available for changes of home address on website . It is therefore requested that please provide a copy of home address changes form and hole document change to our nearest post office so can i eaisly operate the policy
